PTSO Membership Form

Please complete a membership form for each person that is joining the NHS PTSO. This will give us accurate information about all the fun activities we have planned for the year. Membership is $5 per person and your membership isn't complete until payment is received. Please submit your payment by turning in cash or check to the NHS Front Office (labeled with your name) or online through MySchoolBucks.
